Thanks to a $297,000 investment from Health Net and a local health plan, Californians who have health challenges will receive personalized care management, safe housing options, nutritious meals, and other vital services designed to improve health and stability.

The funding supports two programs: Enhanced Care Management (ECM) and Community Supports. These programs help people who need more than basic medical care.

What These Programs Do

  • Enhanced Care Management (ECM): Each patient gets a Lead Care Manager. This person helps them manage their health and connects them to doctors, mental health care, and social services. It makes care easier and more personal.
  • Community Supports: These services help people stay healthy by providing:
    • Safe housing
    • Healthy, medically tailored meals
    • Fixing home problems like mold or pests

Better Technology and More Helpers

The grant will also improve technology, like electronic health records, referral tools, and data tracking. This makes it easier for care teams to work together and follow health rules.

Plus, the money will help hire and train more care managers, coordinators, and community health workers. These people will support more patients with complex health needs.
