A joint partnership between Advanced Health Care Sacramento LLC, and Continuum Care has begun the development of a new 60-bed post-acute transitional care facility in Elk Grove. 

Dr. Shelton Duruisseau, formerly Associate Vice Chancellor, UC Davis Health, and Allen Warren, president of New Faze development, created Continuum Care Hospital LLC in 2018 to develop post-acute care facilities throughout the greater Sacramento region. 

Groundbreaking for this new facility, Advanced Health Care of Elk Grove took place Nov. 15 at the intersection of Lotz and Whitelock Parkway. It is expected that the facility will open in fall 2024.

Advanced Health Care of Elk Grove represents the first project between Continuum Care Hospital and Advanced Health Care. This facility is the first of its kind built in Elk Grove. The partnership expects to develop and bring more senior care facilities to market as well as an additional transitional rehab facility in Roseville. At the same time, CNA clinical training programs are being developed to accommodate these care facilities.

Officials say the project will bring much-needed post-acute transitional care capability for patients needing short-term skilled nursing, rehabilitation services following surgeries, acute incidents, and exacerbation of chronic diseases throughout the southern part of Sacramento County as well as Lodi, Galt, and Stockton communities. Once completed, the facility will offer state-of-the-art inpatient physical therapy, occupational therapy, speech therapy, nursing, and dietary care to patients who need intensive rehabilitation prior to transitioning back home. 

The facility will be operated by Advanced Health Care, a national post-acute operator headquartered in Farmington, Utah. Advanced Health Care operates five-star facilities across the United States, including Advanced Health Care of Sacramento.

Advanced Health Care of Elk Grove will feature 60 private patient rooms, including en-suite bathrooms and high-end finishes. In addition to intensive and innovative rehab regimens, patients will experience first class dining and activities in a restful home like and rejuvenating atmosphere. 

Dr. Duruisseau and Warren are determined to help meet the growing demand for seniors and housing. Warren has more than 30 years of experience developing residential housing. Duruisseau is a 40-year veteran in the healthcare industry, and formerly an Associate Vice Chancellor, UC Davis Health. After he retired from UC Davis Health, he created and developed Quantum Care Place LLC that owns and operates an assisted living memory care facility in Citrus Heights, with plans to open another facility in El Dorado Hills.
